2011-2012 Health Requirements for students entering 7^th grade:

This is just a reminder that all students entering 7^th grade, Nebraska School Law requires all students to have a physical examination within six (6) months prior to the entrance of school.   A dental examination is also recommended at this time.

All physical forms, immunization updates, or documentation of varicella need to be on file in the school office prior to the first day of school.
